Confidentiality Agreement and Privacy Notice Monitoring and Support Group Member
The privacy notice should be supplied to any party offering to act as a member of a Monitoring and Support group. The confidentiality notice should be signed prior to engagement with the group and returned to the District Safeguarding Officer. July 2018.
Monitoring and Support Group Subject Privacy Notice and Information Receipt
This form should be supplied to all those about whom a safeguarding concern has been raised and as a result are now engaging in a Monitoring and Support Group. It contains a GDPR compliant privacy notice and an information receipt. July 2018.
Safeguarding Risk Assessment Privacy Notice and Information Receipt Contributor
This form should be supplied to any party contributing to a safeguarding risk assessment who is not the subject of the concern. It informs them of their data rights. It also provides an opportunity for the contributor to express communication and support preferences. They should also be supplied with the relevant information leaflet about the process. July 2018.

The privacy notice should be supplied to anyone reporting a safeguarding concern who is not the party to which the concern applies or any party providing information about that concern. SGC/1 Safeguarding Contract Information Summary
This form should be used to collate information to support introduction of a safeguarding contract where there is an automatic requirement under standing orders 010 prior to a party engaging with the Methodist Church. It should be used in conjunction with SGC/2 & 3. January 2019.

To summarise risks to support introduction of a safeguarding contract where there is an automatic requirement under standing order 010 and 690 for engagement in church activities. The summary of risks should be based on information collated via SGC/1. January 2019.
